1/ #Metabolism, #lipids, and #ASCVD
#megathread🧵🧵🧵

Okay, I want to revisit and breakdown my position on this crucial topic and the challenges in communicating it.

⚠️ Get comfortable, as this could get long.

Ready? Let's go...
2/ First, it's worth reviewing why there's a high level of confidence low density lipoproteins (LDLs) drive atherosclerotic cardiovascular disease (ASCVD)

For an excellent, lay-person video, I recommend @NutritionMadeS3's from a couple years ago 👇

3/ Also in the name of arguably the most cited meta-analysis for the Lipid Hypothesis to date:

"Low-density Lipoproteins cause atherosclerotic cardiovascular disease"

This meta combines lines from genetic, observational, and interventional studies.

1/ My thoughts on #Satiety & @DietDoctor 🧵

If you've followed me a while, you know I try to avoid the "diet debates" (tho not always successfully)

However, I have been getting a high frequency of DMs/messages/calls regarding DD's new endeavor for a #lowcarb alternative
2/ Let me first start off by saying I don't think any diet is inherently superior to all others.

I *do* think the #lowcarb diet is under-utilized for populations that can uniquely benefit from it.

However, I also believe most of us have many more options than is often assumed.
3/ Moreover, I consider @DietDoctor's long history of providing a "one stop shop" platform for all things #lowcarb/#keto with high quality guides, videos, and tools to be without equal.

It made it extremely easy to refer everyone in DD's direction, particularly beginners.

Wichtige Studie!!!

🧵1/8 Here's an easy thread for reference on our current papers. 👇👇👇

🔖 Bookmark this!

We cover:
- High #LDL on #lowcarb/#keto
- Lean Mass Hyper-responders (#LMHR)
- The Lipid Energy Model (#LEM)

- And, ofc, our #LMHRstudy underway right now
2/8 The #LMHRpaper was our first describing the #LMHR phenotype

- #LDL #Cholesterol of 200 or higher
- #HDL #Cholesterol of 80 mg or higher
and
- #Triglycerides of 70 mg or lower

Which is actually common for lean, very folks on #lowcarb/#keto folks. doi.org/10.1093/cdn/nz…
3/8 Our second paper brings a case report on a single #LMHR with extremely high #LDLc (~500 mg/dL) for 2.5 years, their eating patterns, and current CT angiography.

(⚠️Please note this is a single case report and should be considered anecdotal, ofc) doi.org/10.3389/fendo.…

1/ Respectful rebuttal 🧵:

I think you make valid points with regard to a diet community having bad actors that can reflect on it, @jerryteixeira -- but I'd push back that one has to tow the party line on high fat and/or high meat in #keto or get hammered.

Some examples...
2/ I myself have brought up #PlantBasedKeto many times over to raise awareness, even though I try to avoid the diet debates. But I've not suffered any repercussions from the keto community.

3/ DietDoctor.com is arguably the largest resource for #LowCarb diet, and in the last couple years have put out a great deal more material on higher protein with lower fat as a diet direction option. Many low carbers prefer this emphasis over higher fat as a %

Does the ketogenic diet impair endurance training adaptations?

A thread 🧵👇

#keto #ketones #lowcarb Image
The ketogenic diet is a very low carb, high fat diet, that claims to improve exercise performance by increasing the use of fat (fat oxidation) during exercise.

2/ Image
This study investigated the effect of 3 different diets during an intensive training period on training adaptations in elite race walkers.

Restricting total carbs has a much bigger effect than just restricting sugars in the first 24 hours

Check out our latest study (led by @AaronHengist) on carb and sugar restriction 🧵 (1/11)

doi.org/10.1007/s00394…

#lchf #lowcarb #carbs #highfat #keto #ketogenic #diet #health
The main aim was to see if restricting sugars or total carbs alters physical activity levels

This is because previous studies suggested fasting can lower physical activity & this can happen quickly

We wanted to know if this was due to carbs or energy

What did we find?
2/11
25 people (15 female) ate 3 diets for 24 hours with a variety of metabolic and energy balance measures

Why do some people "backload" carbs?

What does this mean and what is the science behind it?

A 🧵 1/12

#exercise #carbs #lowcarb #lchf Image
"Carb backloading" is the practice of avoiding carbs early in the day and eating them later in the day, usually after some exercise.

Why would this make sense to do?
2/12 Image
Some rationalise this based on evidence that after exercise, muscle glycogen levels are ⬇️ and muscle glucose uptake is ⬆️.

Ingested carbs can therefore restore glycogen.

⬆️ muscle glucose uptake should mean our blood glucose remains low right?

1/ 🙏 Happy Accident 🧵

I’m incredibly thankful for that day back in March 2015 when I got an A1c of 6.1 for the second year in a row.

Why?

It led to my finding #lowcarb as a possible means to prevent the onset diabetes…
2/ Shortly after starting, I had immediate family members with health issues be inspired to do likewise.

One of those immediate family members had an A1c of 8.1 at the time. Two were hypertensive, and taking medication for it. And these are just the immediate family members…
3/ off the top of my head, I think there are roughly 2 dozen people between both close friends and family who are sustained lower carb now than they were before For weight loss, or other reasons of health gain.

To be sure, it hasn’t worked for everyone, nor would I expect that.

1/ Two weeks ago we released our paper on the #LipidEnergyModel (#LEM) along with our video abstract for it. I'm pleased to say it has led to many great connections and expanded discussion.

I'm going to recap on a lot of these in this thread. 🧵 ...

2/ First and foremost, thanks to everyone for their extraordinary support in retweeting our announcement, sharing our paper, and letting researchers know of this model.
3/ As we state many times (including within the video abstract), this model doesn't describe all possible influences on cholesterol levels. For example, other things can impact LDL-C such as M/PUFA-to-saturated fat composition, fiber, genetics, medication, etc.

WOW! >300 retweets in first 24 hours!

1/ Now for a thread from the first author on the #LEM Paper:
👉What it IS
👉What it ISN'T
👉👉and What's NEXT...
doi.org/10.3390/metabo…

Read on... @realDaveFeldman
2/ What it IS:
We provide the first description of the long-awaited #LEM hypothesis, a model that attempts to explain:

(i) the inverse association between BMI and LDL-C on #lowcarb diets

(ii) the #LMHR phenotype, in which lean people on low carb exhibit very ⬆️LDL-C⬆️HDL-C⬇️TG
3/ These phenomena, presently, have no complete competing explanation:

If effect were purely genetic, the🧬s would need 2explain:
>how macronutrient change can alter, not just LDL-C, but HDL-C+TG as part of a triad
>correlation between BMI and lipid changes

That's a tall order

#BigPharma har fuld kontrol over diabetes-området.

sebastianrushworth.com/2020/12/02/can…

1/ It's been quite a week on Twitter regarding #lowcarb and LDL

I have been on the fence about weighing in, not wanting to add to the dog pile, but decided 2drop my thoughts in this thread

If u don't have time to READ THE WHOLE THING IN CONEXT, ignore& consider me🪰 on the wall
2/ I'll first address @deirdre_tobias analysis of LDL and all cause mortality. With the caveats that I'm not an epidemiologist &hope to have more insightful questions when time passes and the dust settles, I am not in disagreement w/ the main results: high LDL associated with ACM
3/ That's actually old news, isn't it?Not so say that the analysis doesn't bring something to the table, but on a population level v high LDL levels (will get to low LDL in a minute) are generally undesirable. Thus, all things being =, a conservative gambler would pick low-is LDL

1/ If you want to hear a great discussion on the #LMHRstudy from my colleague,@DrNadolsky (who is most definitely pro-#LDL #Cholesterol lowering), I highly recommend this podcast that just dropped via @Plant_proof.

I'll add just a few quick thoughts...
2/ Firstly, this may be the most I've heard myself talked about in a podcast I wasn't in (😂 weird, but understandable given the context).

But while certainly critical in many respects, I felt both @DrNadolsky and @Plant_proof provided a lot of good context...
3/ @DrNadolsky bring up my oft-repeated phrase, "cautiously optimistic" [in the context of higher LDL/ApoB as a seemingly resulting from metabolic fat-adaption]

Worth repeating that mine is a hypothesis and should be treated as such, hence the "cautious" part of the phrase...

1/ #Keto > #LowCarb

It’s a ironic, my bloodwork looks fine when I’m either keto (less than 25 net carbs) or low carb (25-100 net carbs).

I have a lot more choices when I am low-carb and the food experience is generally more pleasurable with more treats and greater variety…
2/ But in all that time outside of when I’m eating, I just typically feel better. More balanced, better focus, and on top of all of that, I find I simply think about food a lot less. While it’s fun to anticipate a good meal, it can also be an unhelpful distraction.
3/ It’s annoying to have to identify food that is satisfying, but not *too* satisfying. Food I can enjoy but won’t actively overeat. That’s pretty much everything super low carb that isn’t highly refined (I could definitely drink way too much heavy whipping cream “fat shakes”)

(1/n) Some ideas on the planning of a healthy low-carb plant-based diet.

(2/n) ⚠️TWITTER IS NOT A SOURCE OF MEDICAL ADVICE⚠️

• If you have a serious medical condition &/or take medication, it is necessary to consult your doctor before making changes to your diet.

• Ketogenic diets have been associated with serious side-effects.
(3/n) During the first 7 days on a low-carb diet I suffered: head/muscle aches, hunger, fatigue.

AFAIK alleged ‘cures’ for so-called 'keto flu' haven’t been empirically tested.

I did keep hydrated (but this is *generally speaking* a good idea anyway).

1/ How much does "Dietary Cholesterol" impact blood cholesterol levels?

This has been making the rounds lately and I have an experiment from 2018 is key to this question 👇

Remember the "White Bread and Processed Meat" experiment? cholesterolcode.com/the-tandem-dro…
2/ If you've followed me a while, you know I believe it isn't the actual ingestion of cholesterol on #LowCarb that has a big impact on serum levels -- it's predominantly the trafficking of fat. (See CholesterolCode.com/model)

But this is actually a very testable distinction...
3/ All we need is an experiment where I consume a lot of (1) low fat food that still (2) has ample amounts of cholesterol.

Thus, the "processed meat" side of the experiment is key if it has a decent amount of cholesterol -- and it does.

For example 9/21/18 I ate the following..

1/ Long running thought exp'nt:

If a drug did many of the same things with one's lipid profile as a #LowCarb diet, particularly decreasing triglycerides while increasing #LDL #Cholesterol:

1) Would it reduce ASCVD risk?
2) And if so, would it be more acceptable than #LowCarb?
2/ If you haven't heard, SGLT2 Inhibitors have recently emerged and have been gaining a lot of popularity. Data thus far shows they typically:

1) Decrease TG levels
2) Increase #LDL #Cholesterol levels
... yet...
3) Reduce risk for cardiovascular disease
3/ Naturally, that has the attention for those of us interested in how triglyceride trafficking and turnover could be relevant to #LDL increasing for what may turn out to be non-pathogenic reasons (kinda my focus ;) )

But wait -- it get's better...

We know this 2013 dietary guideline does not work.
Please do the NHMRC survey in the next 24 hours and make a case for reviewing #Meat #Salt #Cereal #Fat in 2021. @DrPeterBrukner @MueckeDr & I have drafted a letter with options for you - follow the thread.
surveymonkey.com/r/GTP8G6T?fbcl…
A review of the Australian Dietary Guidelines has commenced and we are writing to inform you on how you can participate.
Together, we will be advocating for a review process that is independent, free from conflicts of interest and relevant to all Australians.
It’s vital we call out the influence of commercial interests in the procurement of scientific evidence that impacts our health.
The scoping survey is the first step, and it closes on 3pm AEST, Monday 15 March. It has two key questions that will drive the direction of the review.
