I'm 1 of 50 million Americans living with an immune system that mistakenly attacks/damages healthy tissue (rude). During #AutoimmuneAwarenessMonth we strive to promote better understanding of these complex chronic conditions. Follow me as I share my journey this month.

3) This is me at 32, a month before I woke up 1 day + couldn't walk. Days later I couldn't bend my fingers. Bilateral pain/swelling + fam history of #rheumatoidarthritis prompted labs. Sky high CRP, ESR, rheumatoid factor sent me to a #rheumatologist.

#pleuraldisease is fascinating and nuanced. First step is #lightscriteria which give favor to finding #exudates which tend to be more urgent. You only need one criterion, which maximizes #sensitivity. Meaningless statement: "It's an exudate only by protein." One criterion=exudate
(Like on Tinder, the more criteria you require, the more you narrow your pool, compromising sensitivity for specificity.) Transudates I will skip over. Most common #exudate is #parapneumoniceffusion #PPE. If there is or is likely to be #pneumonia, it's PPE. Next task is to...
One of the most upvoted comments on my #ACR21 plenary was "Why are we still doing observational studies on risks of #steroidsinRA? Given unavoidable confounding, shouldn't we just focus on clinical trials?" I responded in-platform, but thought the topic called for a🧵also. 1/
First of all, clinical trials on this topic are very important, and, when well-designed, will almost always yield results more reliable than those of observational studies when powered to look at common, short-term steroid side effects. 2/
However, many steroid-associated AEs are low-prevalence & require long-term followup to detect. MACE, major fractures, sepsis, VTE, etc. It would be very difficult to power an RCT to reliably assess effect of steroid use (esp. low-dose) on these outcomes. 3/

I deeply relate to this. After almost dying of ❤️ failure from #Lyme & being told by 11 “top” docs that in spite of my tick bite, bullseye, & classic symptoms, I could not poss still have it after a short course of doxy. Well, I did. 1/
And after the 12th doc saved my life (thx @StevePhillipsMD), I wrote my story in @HuffPost. I didn’t think anyone would care. But they did. The editor called & asked me to keep writing about this scandal as so many ppl were reading my story. 2/ huffpost.com/entry/how-11-t…
