1/ The issue in these cases is when a medical provider who prescribes controlled medications like opioids for pain or an opioid use disorder (but also meds for seizure disorder, depression, ADHD, etc.) can be held criminally liable for drug distribution.
2/ In simple terms, it comes down to when a doctor is acting as a drug dealer versus as a clinician. But the devil is in the details re: how the relevant statute (Controlled Substances Act) and regulation are interpreted.

2/ ā¢Use of #buprenorphine by a person who has #OUD but is not in withdrawal can precipitate withdrawal. This is a significant adverse experience. It also dissuades patients from adherence to treatment.
3/ ā¢It is unclear whether removal of the X-waiver requirement for #buprenorphine-prescribing will increase access to care in the community. Already, physicians who have an X-waiver do not see the full allowed complement of patients who have #OUD.

MN #opioid Work Group created Taper Guidance. It will be out 4 public comment soon. Some areas extremely problematic. Although it states NOT 2 taper solely 2 meet system or state policy; MN Quality Improvement requires Drs 2 meet MME thresholds = TAPER TO MEET STATE POLICY. š¤
If #ChronicPain pt wants 2 try 2 taper from #opioid analgesics 4 any reason & CONSENTS, there needs 2 be safe way 2 do it. Problem throughout U.S, incl #Minnesota; is non-consensual tapering. Many experts, incl #addiction specialists, stress dangers & even state it's UNETHICAL.
August 31 is International #Overdose Awareness Day. Every year I reflect on all the people weāve lost, but especially Arielle. Here we are at 18 - careless, happy assholes. She was brilliant, snarky, a talented musician, and she died at age 26 from a poisoned drug supply.
In the last 2 years of her life, Arielle went to jail, went to rehab, and āgot clean.ā
Jail didnāt save her.
Rehab didnāt save her.
Abstinence didnāt save her.

So much energy has gone into research looking to define optimal strategies for opioid tapering where is the energy being given to development of other medications that are as effective as opioids with less risk? I know this paper referenced that many #patients tapered 2/

2/Why it matters. Current rules reward caring for tech-savvy patients with 2-way audio-visual. Most of my formerly homeless patients >50 lack equipment, data plan, or comfort. Now there is payment parity, for SOME services. Which ones? Opioid use disorder treatment, for one! #OUD
