Great 🧵- my takeaway since having a 👶🏻myself: if a patient comes to the ER postpartum, DO NOT BLOW THEM OFF. The barrier to leave the house is so high, that if they put on pants to come to the ER, they NEED something. HELP.
Women’s symptoms are often seen as histrionic or atypical at baseline by a medical system grown in patriarchy. Postpartum is one of the most vulnerable times in a woman’s life, so if they seek help, do so, without judgement or barrier.
The issue may be social or a knowledge gap or a lack of healthcare access, but it’s our duty to counsel/reassure/coordinate just as much as it is to treat a classic medical complaint.

I'm off on a work trip -- first trip since baby. Since I'm a #breastfeeding mom, thought I'd keep a thread of my experience pumping during travel since it's something I wanted to see while researching this ahead of time. Here we go!
I fly out from Asheville and North Carolina has basically no regulations for pumping/#breastfeeding moms aside from not arresting them for it. So I was pleasantly surprised that there was a @MAMAVA lactation pod in the AVL airport. My first one, what luxury!
Waiting to board so here are some pumping basics: the best (i.e. most powerful) pumps plug into a wall, so a good breastfeeding spot has an outlet. There's also usually some cleanup after - you're technically supposed to wash all pump parts every time - so a sink is also clutch.

- minimizing their own risk of developing cancer or diabetes
- sensory needs
- bonding with a surrogate child, foster child, or adopted child
- trying to keep a sick child alive

A study published (6th June, 2021) last year in JAMA Pediatrics mentioned that Vaccine-associated mRNA was not detected in 13 milk samples collected 4 to 48 hours after vaccination from 7 breastfeeding individuals.

Recent study published (26th Sept, 2022) in JAMA Pediatrics mentions that of 11 lactating individuals enrolled, trace amounts of BNT162b2 and mRNA-1273 COVID-19 mRNA vaccines were detected in 7 samples from 5 different participants at various times up to 45 hours postvaccination.
WHAT CHANGED?

I’d like to highlight that in the study from 2021 in the conflict-of-interest section, among others, we can see National Institutes of Health (NIH), Bill & Melinda Gates Foundation, CDC Foundation mentioned.

As a haematologist, the most common involvement in breastfeeding is with post-partum anticoagulation advice. DOACs have revolutionised treatment of thrombosis but @ISTH recommend they are avoided in breastfeeding. 1/

I wonder whether this is sometimes interpreted differently. Is it instead interpreted as:
"Breastfeeding is best avoided on DOACs"?
I wonder... are women are put off, or worse - are HCPs advising not to breastfeed so DOAC can be used, avoiding warfarin or injections? 2/
Breastfeeding has so many advantages for mum, baby, and for society but it is not well understood by HCPs. (ncbi.nlm.nih.gov/pmc/articles/P…) so I really worry that this might be the case. The harm that can result from a woman who wants to breastfeed and is told she can't can be huge. 3/

My child was born prematurely in January and spent his first three weeks in the NICU. Premature babies and NICU babies frequently have difficulty latching, and my child is no exception. 15 weeks postpartum and I have never been able to get him to latch without a nipple shield. 2/
Thus I have become an "exclusive pumper." This was not the plan, I was excited about bonding with my baby through nursing. But that quickly became impossible. I was diagnosed with preeclampsia with severe features, and had a traumatic labor and delivery. 3/

We need to end the narrative that breastfeeding depletes mothers. 

Breastfeeding reduces post partum haemorrhage, postnatal depression, heart attacks, strokes, dementia, breast cancer, ovarian cancer, and autoimmune disease.  It's a sound investment and one most families want.
Instead we should focus attention on the midwifery crisis, overstretched GPs and health visitors, the determinants of birth trauma, gender based violence, a paternalistic medical culture, a lack of fully funded maternity leave, health and social inequality, systemic racism,
lack of flexible working, lack of high quality affordable childcare,barriers to parental leave, a gender pay gap, fuel poverty, formula poverty, food insecurity AND a society that doesn't value the caregiving that only they can give.
