Toxicity is most often seen with acetylsalicylic acid (Aspirin), but also methyl salicylate (Oil of Wintergreen- 5 mL = 22 aspirin), bismuth subsalicylate (Pepto-Bismol), teething gels, & headache preparations (BC Powder) ingestion

Uncouples oxidative phosphorylation & ↑Renal NaHCO3 excretion➡️AG Metabolic Acidosis
Stimulates medullary 🫁 drive ➡️ 1° respiratory alkalosis

- Clinical: Tinnitus, N/V, Tachypnea/Hyperpnea, AMS & Hyperthermia (late)➡️⚠️RAPID DECOMPENSATION

Salicylate Lvls:
Acute Tox:
15 - 30 mg/dl = therapeutic range
>30 mg/dl = symptoms begin
> 90-100 mg/dl = severe toxicity
Chronic Tox:
40-60 mg/dl = severe toxicity

🧵 3rd stage of labour
Midwives' scope of practice: MWs sometimes give an oxytocic drug to aid birth of the placenta (active management)
This will be syntometrine or syntocinon depending on area/trust
This is followed by controlled cord traction (CCT)
Normally takes 5-30 mins

Ambulance clinicians can instead use physiology to encourage this process (esp if MW not present)
- Encourage bladder emptying
- Use gravity/upright positioning (have something ready to catch the placenta!)
- Oxytocin helps! (Skin-to-skin, breastfeeding)

25 years ago today I worked my first shift as a Paramedic….25 years!
So young, with so much responsibility.

We are a unique breed in emergency services, the bonds we form can only be compared to those of family. I have had the opportunity to work with some truly amazing people, & am grateful for the role that each & every one of those crazy family members have played in my life /3

Really scary sounding story about #ambulance provision in England today.

Why it's not actually as bad as you first think - a thread by a #paramedic. (1/15)
In the 1970s/1980s, ambulance services were simple; every town had a station, with more ambulances than the town needs.

If you called 999, one of the crews would come, take you to hospital, and then go back to their station to clean, restock, and play pool til next call. (2/15)
Whilst really easy to understand - it only works when you have more ambs than you need, and when it's busy, your two ambulances might both be 'on a job ' and if you call 999, one of the two crews in the next town will have to come - which might be 20 miles away! (3/15)

I was 12 years old when I watched as a man’s wife ran into the restaurant I was at, yelling inside to all, “somebody, please help my husband!”

I saw the man slumped over his steering wheel in the parking lot (likely a sudden cardiac arrest).

I had no idea what to do.

And,
I also knew that that horrible feeling, and not knowing what to do, would NEVER happen again.

On April 19th I enter my 34th year as a #Paramedic. Helping others in the time of most need has been so rewarding.

there was an important development relevant to both Black History & Paramedic History,
back then it was unusual for any care to be given en route to hospital, with most ambulances simply acting to transport patients,

Patients are my greatest teachers & recently I’ve had a patient unexpectedly become one of my biggest cheerleaders - a 🧵

Over the past few mths, I’ve been providing #PalliCare to a patient whose elderly body is ailing them while their mind remains extraordinary & vibrant. 1/
During our visits at their home, I’ve learned that they immigrated to Canada bringing with them diverse life experiences, spending their adult lifetime in academia. They live alone, so upon realizing how big of a nerd I am, they have generously shared their passions & slivers 2/
of their vast knowledge & wisdom with me. Even assigning me homework! Each visit I leave inspired, wishing I had taken notes during these precious conversations. It is an incredible honour to share time with them caring for their body/mind/spirit. I always look forward to it. 3/
