This thread by @conspirator0 gives a good rundown: hashtag starts in Iranian circles, content in Persian and English. Launch time was consistent with a UK location (around 0930 GMT).
The hashtag started here, as a response to @RNCResearch's take on a clip from a Pelosi interview.
The Persian post came first, English explanation over an hour later.

#NancyPelosiFakeNews is trending this morning. We started looking at it a bit before 6:00 AM PST, at which point there were 13129 tweets from 4875 accounts. We've been seeing rumors about bots, but the traffic mostly doesn't look automated.

@ZellaQuixote How did the #NancyPelosiFakeNews trend get started? The early accounts (including @NarimanGharib, who sent the first tweet) appear to be Iranian; tweets are in both Farsi and English. Posting the first tweet in the middle of the night US time likely helped the hashtag to trend.
@ZellaQuixote @NarimanGharib The situation evolved while we were studying the hashtag. Between 6:09 and 6:15 AM PST, Trump retweeted eight #NancyPelosiFakeNews tweets, including an English tweet from @NarimanGharib, the originator of the hashtag. Unsurprisingly, this kicked the volume into high gear.
