Carefully observing a patient walk 20 steps gave clue to diagnosis

1. 1995- I was a first year MD Medicine PG at CMC Vellore.
35-yr old Rami from Midnapore had got admitted with fever of unknown origin of 6 months duration. No cause had been identified despite extensive tests.
2. It was a hectic day with 8 new patients admitted from emergency. It was past midnight, when I managed to examine Rami. During examination, she took a washroom break, for which she walked about 20 steps in all. After she returned to her bed, she was breathless .

3. She was unable to talk in complete sentences. Her pulse rate was 140/min. High #pulse rate could be partly explained by #fever, but it was beyond what was expected for a fever of 102 degrees Fahrenheit (pulse rate can rise by 10 beats/minute per degree rise in temperature).

Giving #Paracetamol in cases of mild #Viral #fever may be harmful
1. It is a common practice to give paracetamol (acetaminophen) to children or adults suffering from viral fever. Research evidence shows that we may be causing more harm than good by this practice.

2. Fever is not a disease; it is a symptom of viral infection. Paracetamol can not kill the virus.
Acute infections, including those due to Corona virus and other viruses, often stimulate a febrile response.
3. A mild fever appears to improve #outcome; as it diminishes viral replication by several mechanisms, including virion entry into host cells and genome transcription, and improving host defense mechanisms against the pathogen.

In the past, too many decisions in #CriticalCare were guided by fear. We oversedated patients for too long out of worry they might self-extubate & pull off restraints.

That’s happening again 🤮
3/ We kept patients immobilized due to an overly conservative approach to fall prevention that precluded us from attempting to walk them early. We kept families away from patients, treating loved ones as visitors rather than members of the healing plan.

That’s happening again.

Fever in pregnancy is potentially a concern because a having temperature of more than 39 C (102 F), lasting for at least 24 hours, during the first trimester is associated with an increased risk of certain birth defects. 2/

Thermal scanners finally getting attention for their inaccuracy in screening for #COVID19. I’ve covered thermography in detail over the pandemic in an effort to raise awareness about the differences in how #fever presents by age and even time of day. –a thread--

2/ I am an active and healthy 26-year-old. The type of person people say “shouldn’t be stressed” about this virus, much like our students. COVID-19 is a sickness unlike any I have ever endured. The weakness my body felt was worse than any flu, cold or stomach virus I’ve ever had.
3/ My symptoms started slowly, a slight cough, that sent chills down my spine in fear that I had contracted the virus. I immediately quarantined. That evening I started having chest pains similar to heartburn, and the next morning I awoke with a #fever.

Did you know fever is a healthy immune response and temperature upto 104°c is fine. Fevers turn on the body's immune system. They help the body fight infection. Normal fevers between 100° and 104° F (37.8° - 40° C) are good for you. (1/n) #Fever #Myths thread
MYTH. Oral temperatures between 98.7° and 100° F (37.1° to 37.8° C) are low-grade fevers.
FACT. These temperatures are normal. The body's normal temperature changes throughout the day. It peaks in the late afternoon and evening. A true low-grade fever is 100° F to 102° F
Fevers with infections don't cause brain damage. Only temperatures above 108° F (42° C) can cause brain damage. It's very rare for the body temperature to climb this high. It only happens if the air temp is very high. An example is a child left in a closed car during hot weather.

Fever (pyrexia) is an elevated core body temperature as part of a physiological response to infection controlled by the hypothalamus

This is opposed to hyperthermia, a pathological elevated temperature

This is important, as your body is IN CONTROL. Fever happens on purpose

What temperature counts as a fever?

There's no strict definition (there's no really good data on what a "normal" temperature is...), but an arbitrary cut of off 38C is often used.

Its better taken in context; 37.8C following temps >38.5C is probably still a fever.

Very interesting! 👉 Team found (in mice) that #fever alters surface proteins (integrins) on #immune cells such as lymphocytes + macrophages ☝️ These changes allow such cells to better travel via #blood vessels to reach sites of infection.
And yet, if you walk into a USA #pharmacy, most medications for flu, colds, inflammation, injuries etc deliberately “knock down” #fever to temporarily palliate symptoms 👉 How do such #medications then impact long-term immunity, pathogen persistence and #microbiome health??
This same trend (medications that treat the #immune response as an “enemy” vs. an “ally”) are also “standard of care” for chronic disease 👉 But as such “#autoimmune”/inflammatory conditions are increasingly tied to #microbiome dysbiosis does that paradigm still make sense??
