The farce that is #facecoverings & #facemasks against #COVID19 , a thread.
First up, #WHO, in latest ‘interim guidance’ of 1 Dec, on ‘potential benefits & harms’ of #masks: apps.who.int/iris/bitstream…. (note 1st ‘potential benefit’ is ‘reduced spread of respiratory droplets’ - which is not actually the same as reducing transmission, is it?) Image
But you’re convinced the benefits outweigh the harms and go for masking. I mean, forcing people to keep a piece of fabric/paper and plastic on their faces, how hard can that be, eh? Well, here’s the WHO’s most recent interim guidance again. It’s not as simple as it looks, is it? Image

A descriptive analysis of existing literature presented a #SARSCoV2 risk-chart from asymptomatic people in different settings and for different occupation times.
Lead authors (@trishgreenhalgh) noted that this is an indicative table; and not quantitative or predictive. Image
Great overview of occupancy (indoors/outdoors), ventilation (well/poorly), type and level of group activity (silent, speaking, shouting), time duration and face coverings --> and risk of #SARSCoV2 transmission.
Variation in susceptibility, viral shedding rates were not looked at.
An indicative, traffic-light like chart for an overview of transmission risk. Other take home messages:
1. Distribution of viral particles affected by air flow too.
2. #SARSCoV2 may travel >2m through coughing,shouting.
3. Rules should reflect ventilation, occupancy, exposure time.

Another hasty & necessary U-turn by government, but once again ill-thought out. Only mandating masks in schools in “high risk” #COVID19 areas will create divisions & stigma between pupils & areas. And it’s *not preventative* - once again waiting for an outbreak b4 taking action.
Where is the government's *public information* campaign with #facecoverings? Unless it is worn properly, stored safely, washed frequently AND in addition to handwashing, social-distancing measures it will not be effective in reducing coronavirus transmission. @IndependentSage Image
It is unfair of Boris Johnson to leave it to headteachers to assess whether #facemasks should be worn by 2ndary pupils in areas where it’s not mandatory. Where is the government’s leadership on #COVID__19? Why is the government not sharing the science with schools? #BackToSchool

1-Thank you BMJ.
Risk of airborne transmission must be considered even in absence of use of AGPs
It maybe as important if not more as a route of transmission in close encounters (˂ 2m) as droplet&fomite routes
@KailashChandOBE @petermbenglish @raj_psyc

2- Recent paper👇
short-range airborne route was found to dominate at most distances(˂ 2m) studied during both talking&coughing
The large droplet route only dominates when the subjects are within 0.2 m while talking or 0.5 m while coughing
@petermbenglish @PPEorg @doctorshaib
3-Authorities must now take airborne transmission seriously&pay more attention to ventilation in closed spaces
#socialdistancing #handhygiene #facecoverings remain important but don't forget #ventilation

Debate (#NL) over face covering or not. On #RIVM/#OMT's advice, @MinPres and @hugodejonge from @MinVWS have unequivocally told us that there is little conclusive evidence (agree) and masks do not work (do not agree).
Great @bmj_latest article, a summary:
bmj.com/content/370/bm…
Face masks have been framed largely as a medical intervention.
Many countries/agencies now mandate/recommend masks against #covid19.
Narrative of face masks as a medical intervention ignores the social practice of this act, and the behaviours associated with it.
Masks as medical intervention (1/2):
Almost all evidence comes from healthcare facilities and addresses the efficacy in protecting the wearer from infection, not as source control.
Using evidence based from healthcare and using it to address community settings? Apples and Oranges

Love them or loathe them, face coverings have been used to protect us from all sorts of harmful material in the air for a very long time. [Thread] #facecoverings #facemask Face covering with scissors. © Lizzie Enfield.
Face coverings as a way of protecting respiratory health go back at least 2,000 years. How UK’s police will tackle face #mask dodging as doctors slam 'illogical' messaging - Senior medics have criticised the "illogical" messaging around face coverings which could risk the spread of coronavirus .

The British Medical Association (BMA) said a second peak of Covid-19, combined with a seasonal outbreak of flu, could be "devastating for the NHS" and voiced criticism of Government guidance on the use of face coverings.

The fears come as a police force revealed they will not respond to calls simply about people not wearing #masks
#Facecoverings are already compulsory on public transport in England & Govt has made it mandatory to wear them in shops from Jul 24 to reduce the spread of #COVID19 3/3

Source of this is a report released today by Academy of Medical Science: Preparing for a challenging winter 2019/20.
Sobering reading and a realistic assessment IMHO.
Recovery planning taking place now must incorporate the v real likelihood of a COVID19 + Flu wave in the winter.
They identify 4 challenges:
1. Large resurgence of COVID-19 nationally, + local/regional epidemics. Peak in hospital admissions & deaths Jan/Feb 2021, similar magnitude to 1st wave, coinciding with period of peak demand on NHS. ~119,900 deaths, over double number in 1st wave.
2. Health & social care systems disruptions due to reconfigurations to respond to & reduce transmission of COVID-19 + knock-on effect on ability of NHS to deal with non-COVID-19 care. Remobilisation of resources as per 1st wave unlikely to be possible due to winter pressures etc.

Four months after Caprice said it & three months after Gary Lineker said it, the govt finally see sense. #CapriceWasRight
This was @OliverDowden telling @piersmorgan that the reason for finally banning mass gatherings & closing pubs was that “the science had changed”.

This is not true.

The science has remained the same.

It tells us if we do not take measures (e.g. set out by WHO) people will die.
A guide to face masks.

Which one should you wear?

Thinking about this a bit more - the #smokefree #facecoverings comparison. I do think it is relevant, but with caveats. In March 2016 I visited a main railway station in Scotland the day before and the day after the smokefree law came in. 1/5
The day before, people were smoking on the concourse, in cafes at the station and elsewhere. The next day, no one I saw was smoking. Not a single person. The difference was sudden and really noticeable. Plus in time, data showed that overall compliance was strikingly high. 2/5
Last week I visited a local large supermarket. I’d say fewer than one in five people were wearing face coverings. I went back just now, the 2nd day of it being mandatory. Almost everyone, with a couple of exceptions, was wearing a face covering. 3/5

Boris Johnson is not a ‘bumbling loveable buffoon’.

He uses this persona as a front for deflecting scrutiny, avoiding questions & building his Teflon brand.

At this time more than ever, we need serious journalism with journalists to ask serious questions about serious issues. Image
GLEN OWEN: So you’re feeling fully recovered, Prime Minister?

JOHNSON: Braaaah! Phwaar! I can jolly well show you if you like!

How on earth does an interview with the PM at a time of national crisis & what should be national mourning, end up with @BorisJohnson doing press ups? Image
Bumbling is his strategy.

“He put on a buffoon mask to become a celebrity, & now he can’t take it off.”
(J Lanchester, 2008)

“He seems to know that if we’re chuckling at him, we’re not likely to be thinking hard about his agenda, or doing anything to counter it.” (J Coe, 2013).

It's a hot topic why I'm not ordering a requirement to wear #masks at this time. To be clear, in alignment with @WHO, @PHAC_GC, & @CPHO_Canada, I absolutely recommend we wear #FaceCoverings when we can't keep 2 m #PhysicalDistancing. Here are 3 reasons why I don't require it: 1/8
1. Enforcement. A public health order requires a court filing and an appearance before a judge every time one wishes to enforce it on a person/business. An order would be basically unenforceable. If we don't enforce, it's effectively just a recommendation (already done that). 2/8
2. Strength of science. While science increasingly supports #facecoverings are likely effective at slowing #COVID19 spread, research is still weak. E.g. @TheLancet systematic review finds much potential benefit, but only “low certainty” that effective. thelancet.com/journals/lance… 3/8
